what is it?
common chronic inflammatory condition affecting the elderly
where does this inflammation usually occur?
muscles around the shoulders, neck and hips
who does it affect?
ONLY women over 50
what is it associated with?
giant cell arteritis
which antibody is associated?
HLA DR4
what is the pathophysiology?
immune cells directly attack endothelium
transmural - all layers are affected
what are the symptoms?
morning stiffness lasting 45 mins
fatigue, anorexia, weight loss, fever
strength is normal
what are the signs?
reduced movement of shoulders, neck and hips (unable to put hands behind back)
normal muscle strength
carpal tunnel
pitting oedema
what is seen if renal artery is involved?
hypertension
how is it diagnosed?
mainly clinically
what investigations can be used?
bloods
biopsy
angiogram
what will be seen on bloods?
raised inflammatory markers - ESR
how is it differentially diagnosed from polymyositis?
creatine kinase is normal in this but raised in polymyositis
what is the management?
prednisolone - usually resolve by end of course
what is the management of non organ threatening disease?
steroids + azathioprine/methotrexate
