What is PMR?
A
Clinical syndrome characterised by pain and stiffness of shoulder, hip girdles and neck.

Who is affected by PMR?
A
- Older ages - average age of onset is 70 years, peak incidence is 70-80 years
- Associated with GCA

What is the history of PMR?
A
- New onset proximal limb pain and stiffness in neck, shoulders and hips (in mornings)
- Difficulty getting out of chair or combing hair
- Night time pain
- Systemic symptoms - fatigue, weight loss, low grade fever

What is seen on examination of PMR?
A
- Decreased range of motion (ROM) of shoulders, neck and hips
- Muscle strength is normal, limited by pain/stiffness
- Muscle tenderness

How is PMR diagnosed?
A
History and Exam supported by raised inflammatory markers (ESR/PV + CRP)

What is the treatment for PMR?
A
- Prednisolone (15mg daily) then tapered down slowly
* Methotrexate can be use as a steroid sparing agent in relapse
