Myeloproliferative disorder of clonal proliferation of a marrow stem cell leading to an increase in red cells often accompanied by overproduction of neutrophils and platelets

Transformation to acute leukaemia (5-15% of patients)

A patient presents with gingival bleeding, confusion and fluctuating GCS. Diagnosed with PV. Recently started on chemotherapy. O/E - pétéchial rash, painless hepatosplenomegaly. Bloods - thrombocytopenia, increased PT and APTT. What is the diagnosis?

A

Patient likely has DIC, due to acute promyelocytic leukaemia
