PNS conduction exam
Sensory/motor/reflex tests in innervation field of suspected nerve
Tests: nerve conduction impulse and not pain
If pain it is not positive for nerve involvement
Principle one: muscle weakness testing
Isometric Tests:
- Nerve root (myatomes)
- PN: all muscles innervated by PN
- Plexus: all muscles innervated by PN from plexus
Maximum contraction
do you resisted Isometric contraction
Oxford scale: weakness due to PN injury
Name three functional test
1) Walking on heels (L5 - toe E / L4- DF) and peroneal n
2) Walking on toes (PF- S2 and tibialis n)
3) Eversion( S1 and superficial peroneal nerve)
What to observe when doing functional tests
Atrophy
weakness
tremor
Fasciculation
Compare to surrounding nerves
For NR: compare to one level up and down
PN: test muscle supplied by suspected nerve
What can weakness indicate
Lesion of spinal canal
lesion of muscle supplied by muscle
muscle lesion
Principal 2:sensory testing
Indicate type of pain
Assess PN and RN
Tests for after interview/UMNL
sensation: Electrotherapy Orthopaedic exam (NR/ cauda ) Neuro exam(PN) Neurological exam (UMNL)
Other sensory tests: CNS
Proprioception
tactile discrimination
