- CTDs/Marfan’s syndrome (tall, thin, big hands, males)
- previous PTX
- smoking
- weed
- scuba diving, high altitudes, flying

Empyema - pus on aspiration, acidic pH, low glucose = medical emergency

- hyper-resonance, tracheal deviation
- shock (hypotension, tachycardia, hypoxia)
