What is Pneumonia?
An acute lower respiratory tract infection. Infection of the parenchyma( terminal bronchioles and alveoli)
Common among the elderly in winter
Pneumonia pathophysiology
Infection of the parenchyma causes inflammation leading to fluid and blood build up in the alveoli
Pneumonia S&S?
SYMPTOMS FEVER - with or without rigors Cough PLEURITIC chest pain that is stabbing on inspiration malaise muscle or joint pain HAEMOPTYSIS SIGNS pyrexia dull percussion cyanosis confusion hypotension high temp tachypnoea tachycardia crackling in lungs
Discuss Pneumonia Management
- Assess and manage any major ABCD
- TC?
- ABCDE with thorough response assess
- Consider alternative care pathways
- Transfer if: confusion, >33breath, systolic <90, 65 years+,
- Think Sepsis?
What are the two divisions of pneumonia?
Community acquired and hospital acquired (hospital acquired = acquired >48 hr post admission)
Pneumonia complications?
Pleural effusion, emphysema, lung abscesses, respiratory failure, septicaemia, brain abscess, pericarditis, myocarditis, scholastic jaundice
What is the Pneumonia CURB 65 Test?
C - confusion (ab mental test <8 U - Urea (not relevant to pre H care) R - Resp >30 B - BP < 90 systolic Age >65
3 points or more indicates severe pneumonia with mortality 15-40%
