Symptoms and signs
Fevers Therefore increased temp Anorexia and weight loss Malaise and fatigue Rigors Cyanosis Bronchial breathing Pleural rub Cough with sputum Wheeze Shortness of breathe Hypotension Increased heart rate and respiratory rate Decreased sats Increased tactile fremitites Dull percussion due to consolidation Tachycardia Haemtoypsis Chest pain pleuritic
Diagnosis
History Examination cxr Bloods and abg Sputum testing for culture Tb screen if recent travel and unvaccinated Serology
Curb65
Confusion
Urine over 7mmol
Respiratory rate 30 or more
Blood pressure. Systolic 90, diastolic 60 or less

If score of3 or more than significant highly
Management of CAP
mild mod and severe severity
low severity- amoxicillin or clarithryomycin or doxycycline
If staph suspected add flucoaxcillin
moderate - amxolicin with clarithromycin
or amxocillin and doxycycline
high severity
benzylpencillin and clarithromycin/doxycycline
length other factors
if s.areus resistant
time length and exception
atypical bbacteria
if s. areus resistant to amoxillicin suspected add vancomycin
7days but if stapylocci -14-21 days
if ligenolla add vancomhycin or quinolone
mycoplasma chhlamydiae add doxycycline
HAP
< 5 days cefutaxmine or co-amoxiclav
>5 days tazobactam and pipercillin
