Mild community acquired pneumonia (CURB score of 0 or 1)
PO Amoxicillin (Doxycycline if penicillin allergic)

PO Amoxicillin and clarithromycin
Severe community acquired pneumonia (CURB score of 3 or more)
IV Co-amoxiclav (and/or doxyclycline)
Non-severe hospital acquired pneumonia
PO Amoxicillin and metronidazole
Severe hospital acquired pneumonia
IV amoxicillin, metronidazole and gentamicin
Common organisms of community acquired pneumonia
Strep. pneumoniae, Haemophilus influenzae (Mycoplasma pneumoniae)
Common organsims of hospital acquired pneumonia
gram negative bacilli, pseudomonas, anaerobes
Atypical pneumonias: Staph. aureus symptoms
bilateral cavitating bronchopneumonia
Atypical pneumonias: Staph. aureus risk factors
PWID, leukaemia, lymphoma, CF
Atypical pneumonias: Staph. aureus treatment
flucloxacillin and/or rifampicin (vancomycin if MRSA suspected)
Atypical pneumonias: Klebsiella symptoms
aspiration, cavitiating pneumonia, often in the upper lobes, often drug resistant
Atypical pneumonias: Klebsiella risk factors
alcoholic, elderly, diabetic

Cefotaxime (or imipenem)

common in patients with CF or bronchiectasis, can be hospital acquired
