Patients who respond to empiric abx therapy should show an improved clinical condition within 48-72 hrs, with resolution of s/s within 5-7 days
considerations for practitioners treating PNA
- the patient’s comorbidities (cardiopulmonary disease)
- the severity of the clinical presentation
- the treatment setting (outpatient or hospital)
rational drug selection for PNA
for healthy adults w/ no comorbidities and no risk factors for antibiotic-resistant pathogens
- amoxicillin 1g TID
- doxycycline 100mg BID
- a macrolide (azithromycin or clarithromycin)
standard tx for infant pts w/ chlamydial PNA
- Erythromycin 50mg/kg daily x 14 days
- Azithromycin 20mg/kg/day x 3 days
standard tx for children with bacterial PNA (S. pneumoniae)
amoxicillin for 7-10 days
- 45 mg/kg/day in 3 equally divided doses
- Or 90 mg/kg/day divided into 2 equal doses
mycoplasma pneumoniae
most common cause of PNA in school-age children and young adults
tx for mycoplasma pneumoniae
