A

2 months, 4 months, then boost at 12 months.

if missed, then give vaccine and boost at first birthday, allowing interval of two months between the two vaccines.

  1. Asplenic/splenic dysfunction
  2. chronic renal disease (nephrotic syndrome, or CKD stage 4/5/dialysis/renal transplant)

What counts as ‘immunosuppressed’ to merit PPV23 vaccine?

A
  • chemo leading to immunosuppression
  • bone marrow transplant
  • HIV at any stage
  • asplenia/splenic dysfunction
  • multiple myeloma/genetic disorder affecting immune system (IRAK-4, complement deficiency, NEMO)
  • systemic steroids >1month (dose equivalent to pred min 20mg/day or for kids<20kg, 1mg/kg/day)

A

PLUS

give 2nd PCV13 booster at least two months after routine PCV13 booster on first birthday.

A

Give additional 2nd PCV13 booster dose. two months later, give single dose PPV23 vaccine.

If PPV23 has been given first, wait 6 months before giving PCV13.

How to manage?

A

regardless of prior PCV13 vaccination, give stat PPV23 vaccine. no additional PPV23 is recommended when reach age 65.

the only exception to above is older children and adults who are SEVERELY IMMUNOCOMPROMISED (multiple myeloma, complement deficiency, bone marrow transplant, acute/chronic leukaemia). they need PCV13, and 2 months later PPV23. If PPV23 given first, wait, 6 months before giving PCV13.

Person with sickle cell disease/celiac disease. Do they need PPV23?

A

Yes, because this can lead to future splenic dysfunction.

as spleen affected, will need not just stat PPV23 but also booster every 5 years.

A

Pre-splenectomy: ideally 4-6weeks, min 2 weeks.

Post splenectomy: delay vaccine till at least 2weeks post op.

A

Pre-chemo/RT: 2 weeks before

Post-chemo/RT: at lest 3 months after completion of therapy, to maximise response to vaccine.

If leukaemia: PCV13 min 6 months after

If bone marrow transplant: PCV min 9-12months after.

premature babies - what timeline to follow?

any precautions for extreme prematurity (<28weeks)?

A

follow chronological age (from time of birth)

If extreme prematurity (<28 weeks), give 1st dose with respiratory monitoring for 48-72hours. if any apnea/brady/desats, 2nd dose should also be under similar resp monitoring. Don’t delay/withold vaccination.
