If hospitalized less than 5 days: ceftriaxone 1.5 q8 + metro 500 q8 (PO: cefprozil 500 bid, metro 500 tid)

If hospitalized more than 5 days OR pseudo+: tazo 3.375 q6 or po: amoxyclav (875-125 bid) but not for pseudo
