Causes of Transudate pleural effusion
Left ventricular failure/heart failure (usually bilateral)
Hypoalbuminaemia (malnutrition, cirrhosis, nephrotic syndrome or general protein loss)
Meigs syndrome
Hypothyroidism
Pleural effusion
Collection of fluid in pleural space
Transudative (protein 35g/L)
Causes of Exudate pleural effusion
Pneumonia (unilateral)
Pulmonary infarction/PE
Rheumatoid arthritis
Malignancy
Appearance of pleural effusion
Clear--> transudate Blood--> haemothorax Chyle/milky --> chylothorax Pus--> empyema 'Anchovy sauce'--> amoebic abscess
CXR findings in pleural effusions
Small: blunt costophrenic angles
Large: water dense shadows with concave borders
With pneumothorax: flat horizontal border
Treatment of pleural effusions
Drain
Talc if recurrent
Neutrophils ++ in pleural effusion
Parapneumonic effusion, PE
Lymphocytes ++ in pleural effusion
Malignancy, TB, RA, SLE, sarcoidosis
Mesothelial cells ++ in pleural effusion
Pulmonary infarction
Abnormal mesothelial cells in pleural effusion
Mesothelioma
Multinucleated giant cells in pleural effusion
RA
Glucose <3.3mmol/L in pleural effusion
Empyema, malignancy, TB, RA, SLE
pH <7.2 in pleural effusion
