Pleural effusions Flashcards
How to confirm pleural effusion
Pleural tap and take out sample
What are the two main classifications for pleural efufsion
Transudate
Exudate
what is transudate fluid
Effusions caused by factors which affect hydrostatic pressure, pleural permeability, and oncotic pressure
What is exudate fluid
Changes to the local factors which influence the formation and absorption of pleural fluid
What causes transudate effusions
Congestive heart failure
liver cirrhosis
Nephrotic syndrome
What causes exudate effusions
Malignancy
Infection
Pulmonary embolus
How to classify effusions
Protein concentration in fluid:
<30g/L is transudate
>30g/L is exudate
Symptoms
SOB
Cough
Pleuritic chest pain
Findings which suggest effusion
Percussion dull
Breath sounds quieter
How to investigate
Resp exam
CXR (blunting of costophrenic and cardiophrenic angles, potential mediastinal shift)
Ultrasound is the most sensitive imaging modality to detect or estimate pleural fluid volume
Complication of pleural effusion
Treatment
Empyema (infection)
Drainage and abx
Investigation for a unilateral pleural effusion
Pleural tap and do a bronchoscopy to get a biopsy to see if there is a malignant cause
