What is the pathophysiology of placenta accreta?
deficiency in the decidua basalis
Define placenta accreta
chorionic villi ATTACH to the MYOMETRIUM
rather than being confined by the decidua basalis
Define placenta increta
chorionic villi INVADE into the MYOMETRIUM
Define placenta percreta
chorionic villi fully PENETRATE MYOMETRIUM + extend THROUGH into the uterine serosa (PERIMETRIUM).
In some cases they can invade into surrounding structures.
Give 4 complications of abnormal invasive placentation
Preterm delivery
APH
Risk of severe haemorrhage
Death (P. percreta 7%)
Why is there a risk of severe post partum haemorrhage with abnormal invasive placentation?
placenta does not properly separate during labour
