pituitary apoplexy is a
endocrine emergency it’s from hemorrhage into a pituitary gland (generally adenoma) and see severe headache, bitemporal visual field deficits, and paresis of the oculomotor nerve 3.
why is pituitary apoplexy an emergency?
catastrophic if not diagnosed and treated appropriately. can have meningeal signs that can lead to misdiagnosis of subarachnoid hemorrhage or meningitis
pituitary apoplexy with acute secondary adrenal deficiency can cause
severe refractory hypotension
treatment of pituitary apoplexy is
immediate high dose glucocorticoid therapy empiric glucocorticoid therapy is recommended in people who have just pituitary apoplexy without signs of adrenal insufficiency given the frequency of adrenal insufficiency with pituitary apoplexy
Diagnosis of pituitary apoplexy is confirmed by
MRI of brain CT scan is less sensitive.
pt has hypotension and diaphoretic. see severe headache that has steadily become worse and it’s dull and diffuse. develops bitemporal visual field deficits, and paresis of the oculomotor nerve 3.
