- Confluent or back to back glands lacking intervening stroma
- Cribriform or microacinar configurations
- Complex papillary, micropapillary or villoglandular structures
- Resembles proliferative type endometrium with varying features/degrees of atypia but cytology must differ from that of surrounding nonneoplastic glands

- Psammoma bodies may be present in up to 33% of cases
- Cytoplasm scant but can be abundant with eosinophilia or clearing
- Tumor cells colonize existing endometrial glands
- Nuclei high grade with pleomorphism, hyperchromasia, prominent nucleoli and frequent mitotic figures (including atypical mitotic figures)
