Hemodynamic changes in pregnancy
- Increased total body water
- Decreased peripheral vascular resistance
- BP drops in second trimester
- Leg edema, particularly with pre-eclampsia
Cardiovascular changes in pregnancy
- Increased cardiac output
- Exaggerated splitting, SEM common
- ECG shows left axis deviation, sinus tachy, mild inferior ST changes
Respiratory changes in pregnancy
Hyperventilatory state
Hematologic changes in pregnancy
Increased RBCs but physiologic anemia
Impaired leukocyte function
Increased iron requirements
Hypercoagulable state
Renal/GU changes in pregnancy
GFR increases (decreased Cr by half!, Urea)
Glucosuria can be physiologic but should test for DM
Increased risk of UTI due to urinary stasis
GI changes in pregnancy
GERD very common
constipation, hemarrhoids, bloating
Liver changes in pregnancy
Increased ALP, decreased albumin (increases bone turnover via feedback with calcium)
Endocrine changes in pregnancy
50-70% reduction in insuline sensitivity
Thyroid: TSH decreases early, increased thyroxine, total T3/4
Fetal shunts
- Ductus venosus: bypasses liver, becomes ligametum venosum
- Foramen ovale: directs oxygenated blood to brain. Closes after birth
- Ductus arteriosus: pulmonary artery to aorta (bypasses lungs). Becomes ligamentum arteriosus
