External inspection of eyes.
Conjunctiva and sclera is clear. No redness or lesions in any aspect of your eyes.
Tested pupillary reflex.

Oral cavity.
Mucous membranes are wet, no swelling, no lesions.
Anterior cervical and supraclavicular lymph nodes.
Symmetric, nontender, small lymph nodes.
Thyroid
Not enlarged, symmetric, no nodules, no tenderness.
Inspect and palpate precordium. Apical Impulse.
Drape properly here.
Auscultate heart with diaphragm.

Auscultate with bell with pt supine.

Assess lower extremities for edema.

Palpate radial, dorsalis pedis, and posterior tibial pulses.

Inspect chest size, shape, symmetry, and uses of accessory muscles or presence of retractions.

Percuss posterior chest.

Ausculate 2 regions in front and three in back.

Speech is clear and coherent.
Cranial nerves
2 pupil reaction to light. EOM, Sensation of face. Close eyes, open mouth, say ah, stick tongue out, shrug shoulders, turn head against my hand.
Strength of upper and lower extremities

Patellar and bicep tendon reflexes

Assessed sensation bilateral lower extremeties.
