A pharmacist must notify the Board in writing, within 15 days, of any change in e-mail address, employment location or residence address.
What are the continuing pharmaceutical education (CPE) requirements for pharmacists (e.g., how many hours per renewal period, how long is the renewal period, are live hours required, any specific CPE topic requirements)?
30 hours of CPE per renewal period.
Required CPE:
Pharmacy law - 2 hours
Patient safety or medication error prevention - 2 hours
Cultural competency - 2 hours
Pain management - 1 hour
The remaining 23 hours should be pertinent to pharmacy.
Renewal cycle is every two years.

If a pharmacist’s license lapses, what action can be taken to reinstate the license?
To reinstate a lapsed pharmacy license, the pharmacist must:
1.) Pay the license fees and delinquency fees for all years during the time the license was lapsed
2.) Provide certification of completion for continuing education requirements for all years in which the license was lapsed
3.) If the license was lapsed > 1 year, pass the MPJE with a score of ≥ 75
4.) Complete an application for licensure, with a fingerprint card or other documentation to conduct a criminal background check
Are there any special requirements to become a nuclear pharmacist?
- Be a licensed Oregon pharmacist
- Meet minimal standards of training and experience in the handling of radioactive materials
- Submit to the Board evidence of current certification in nuclear pharmacy by the Board of Pharmaceutical Specialties, or evidence that they meet both the following:
o Certification of a minimum of six-month on-the-job training under the supervision of a qualified nuclear pharmacist in a nuclear pharmacy providing radiopharmaceutical services; and
o Certification of completion of a nuclear pharmacy training program in a college of pharmacy or a nuclear pharmacy training program approved by the Board. - Receive a letter of notification from the Board that the evidence submitted by the pharmacist meets the above requirements and has been accepted by the Board.
What are the requirements to serve as a pharmacist preceptor?
1) Must register with the state board
2) Must have a year of experience as a practicing, licensed pharmacist
3) Must notify the school of pharmacy if precepting students from more than one school at the same time

2) With the written approval of a school of pharmacy, a preceptor may supervise up to 10 interns at public-health outreach programs (e.g., informational health fairs that provide general information but not direct patient care).
3) An immunizing pharmacist may supervise up to two immunizing interns at an immunization clinic.
