3.) If the license was lapsed > 1 year, pass the MPJE with a score of ≥ 75
4.) Complete an application for licensure, with a fingerprint card or other documentation to conduct a criminal background check

  1. A sign visible to the public saying, “This pharmacy may be able to substitute a less expensive drug which is therapeutically equivalent to the one prescribed by your doctor unless you do not approve.” The print must be in block letters and not less than one inch in height.
  2. Providing notification in multiple languages of the right to free, competent oral interpretation and translation services, including translated prescription labels, for patients who are of limited English proficiency.
  3. A sign at the entrances stating the pharmacy hours of operations when a pharmacist is not in attendance if the pharmacy operates as a double set-up pharmacy.
  4. If the pharmacy provides naloxone services, there must be written notice in a conspicuous manner that naloxone and medical supplies to administer are available.

The pharmacy must be notified when the kit has been opened or is expired, and a pharmacist must restock and replace it within a reasonable amount of time.

    o Ensuring the patient has had a clinical visit with a healthcare provider for reproductive or sexual health in the past 3 years
    o Provide the patient with a written record of the hormonal contraceptive administered, or the self-administered contraceptive that was prescribed and dispensed
