A

If the license has been inactive, delinquent, suspended or revoked for < 1 year, the pharmacist must:

1) Provide written notice to the board requesting an active license

2) Satisfy all past due CPE as required by the board; and

3) Pay all cumulative license renewal fees and any applicable penalty fees for the period during which the license was inactive

If it has been 1-5 consecutive years, the above must be completed, plus a period of pharmacy internship (hours depend on the length of time the license was not active) and completion of MPJE.

If it has been > 5 consecutive years, the above must be completed plus the NAPLEX.
