They are ultrametabolizers of it so they get a lot less benefit from smaller doses

Inhibits vitamin K epoxide to prevent thrombosis, CYP2C9 and VKORC1

Why is warfarin so difficult to give?
A
You have to check it regularly to make sure it is enough to prevent thrombosis but avoid excessive bleeding
