Pharm: Laxatives & Anti-Diarrheal Agents Flashcards
What is an osmotic laxative?
Nonabsorbable substances that draw water into the intestinal lumen leading to distention leading to peristalsis
What are the two main magnesium-containing diuretics?
- magnesium citrate
- magnesium hydroxide (Milk of Magnesia)
(Think of the Mg2+ magnets used to massage surfer dude)
___________ is often used in pre-op settings to clean out the colon.
PEG (polyethylene glycol)
Think of the wooden “peg” uncapping the drain letting the bubble bath water to flow out
What is PEG?
An osmotic laxative- an nonabsorbable sugar
How does lactulose work?
Colonic bacteria metabolize lactulose into acid, lowering colon pH, converting NH3 to NH4+, trapping it in the lumen for excretion (think of the pH meter on the fish tank)
What are two non absorbable sugars?
- PEG
2. lactulose
What are two triggers of hepatic encephalopathy?
- Colonic bacteria making NH3
2. Eating too much protein
__________ is used in killing bacteria that generate NH3 in hepatic encephalopathy.
Rifaximin (think of the Rifaximin Fisherman)
What are three types of laxatives?
- osmotic
- bulk-forming
- surfactant agents (stool softeners)
- stimulant (cathartics)
Two bulk-forming laxatives are ________ and __________.
methylcellulose and psyllium- both are large molecules that are unabsorbed by the GI tract. They can also absorb large amounts of water (think of the bulky seaweed pile)
How does docusate work?
It is a surfactant agent that facilitates water penetration into the stool (think of the water-logged canoe at the DOCK)
Chronic use of _________ can lead to characteristic darkening of the colon.
senna - effect is called melanosis coli - stimulates enteric nervous system (think of the senna sun tan being rubbed vigorously onto abs)
What are two contraindications of antidiarrheal use?
- bloody diarrhea (bloody stool OUTSIDE the massage parlor)
2. fever (inflammatory candles OUTSIDE the massage parlor)
What does stimulation of the mu-opioid receptors do?
It increases segmentation (NOT peristalsis), thus increases transit time.
What are two examples of antidiarrheal agents?
- mu-opioid agonists (think mu-ssage)
2. octreotide
