Aldosterone Antagonists: Spirinolactone and Eplerenone
A
MOA: Block Aldosterone receptors in the heart and vessels Diuresis: off load small amt. of volume
Normally- Role of Aldosterone in Heart Failure…can be harmful
Promote renal retention of sodium and water
Promote myocardial remodeling => worsens HF
Promote myocardial fibrosis =>dysrhythmia
By blocking: Reduce Na and water retention , decrease volume
Counteract loss of potassium
Can reduce symptoms and decrease hospitalization
ADR:Hyperkalemia
Care if used with ACEi
Current guidelines suggest adding Aldosterone Antagonists to patient with persistent symptoms despite adequate treatment with ACEI, BB and Diuretic
