Describe what is occuring during cervical ripening
Initially, the cervix contains a densely packed collagen fiber plug that prevents the fetus from leaving the uterus
During cervical ripening, the plug becomes loosely associated

What are the drugs that are used to stimulate the uterus called?
Oxytocics
MOA of misoprostol?
Prostaglandin E1 analog
Clinical indications for misoprostol?
- Termination of intrauterine pregnancy
- OFF LABEL = cervical ripening
Contraindications of misoprostol
Pregnancy (unless aborting)
Previous cesarean delivery (disrupts uterine scar leading to rupture)
AE of misoprostol?
MATERNAL = N/V, chills, shivering, Tachysystole
FETAL= hypoxia due to prolonged uterine contractions
MOA of dinoprostone?
Prostaglandin E2 analog
Clinical indications for dinoprostone?
Cervical ripening
Induction of labor
Contraindications for dinoprostone?
Pregnancy (unless aborting)
Previous cesarean delivery (disrupts previous uterine scar)
Maternal AE for dinoprostone?
N/V, fever, chills
During abortion, fever unresponsive to NSAIDS
Fetal AE of dinoprostone?
Hypoxia
MOA of carboprost?
Prostaglandin F2alpha analog
Indications for carboprost?
3rd line treatment for
Inducing abortion
and
Post-partum hemostasis for refractory bleeding
Important to note about Maternal AE of carboprost?
TONS of AE, pt will feel awful after taking
Hypertension, pulmonary edema, chills/shivering, dizziness, gagging, heartburn, etc…
MOA of oxytocin?
Posterior pituitary hormone
Clinical indications of oxytocin?
Induction of labor
AE of oxytocin?
Water intoxication (rare)
Contraindications for oxytocin?
DO NOT INDUCE LABOR IF…
Lungs are not mature
or
Cervix is NOT RIPE
What is the historic drug used to induce labor?
Ergot alkaloids
What is the issue with using ergot alkaloids?
Causes prolonged tonic uterine contraction
CONSTRICTS aterioles and veins
What are two conditions that would warrent pre-term delivery?
HELLP (hemolysis, elevated liver enzymes, low platelet count)
Pre-eclampsia
What is the function of fetal cortisol?
Drives lung maturation
What are the two options to give to drive fetal lung maturation?
Bethamethasone
Dexamethasone
What is magnesium sulfate used for?
Prevents eclamptic seizures
MOA of terbutaline?
Beta2 agonist
MOA of nifedipine?
Calcium channel blocker
What are the BEST choices to use as a tocolytic in the USA?
Nifedipine
Indomethacin
MOA of alprostadil?
Synthetic PGE1
