dinoprostone/Cervidil
Prostaglandin. Cervical ripening. Used to stimulate contractions of the uterus, thereby ripening the cervix. Must wait at least 30m after removal to start pitocin. Left in place for 12 hrs.
Misoprostol (Cytotec)
Used to stimulate contractions of the uterus, thereby ripening the cervix. Must wait at least 4 hr after last misoprostol dose to start pitocin.
Contraindications for misoprostol
previous c-section or more than 6 contractions in 30m period
dose of oxytocin (Pitocin)
increase by 1-2 milliunits q 30m to achieve adequate contraction pattern. IV or IM. Contraindications are non reassuring FHR or tachysystole

NSAIDS during pregnancy
contraindicated. increase risk of pulmonary hypertension in newborns. Inhibit prostaglandins needed to keep open the patent ductus arteriosis in the fetus
Epidural versus spinal
Epidural: hight dose, onset slow, does not cause significant neuromuscular block, multiple dosing possible, can be given at various points along the backbone. Spinal: low dose, fast onset, does cause significant neuromuscular block, single dose, can only be given at specific point along the backbone to avoid damage to the spinal cord
used postpartum to help produce RBC to make up for blood loss
ferrous sulfate (iron)
stool softener to help prevent straining, especially in clients with tears/lacerations
docusate sodium (colace)
treats severe bleeding after childbirth
Methergine, hemabate, misoprostol
