what is the main moa of COCs
inhibit ovulation
alter the lining of endometirum
what does progestin do specifically?
thickens cervical mucus
red. activity of cilia in fallopian tubes
thin the endometrium
what is important about POPs?
need to be taken at the same time every day to keep efficacy
*they do not appear to dependently inhibit ovulation
what is monophasic COC?
contains the same amount of hormone in each tablet
what is the multiphasic COC?
contains varying amount of estrogen and progesterone to try to mimic natural fluctuations
what estrogen is most used in COC?
ethinyl estradiol (mestrandol is the prodrug)
what is special about drospirenone?
only progesterone with antiandrogenic effects
assoc. w/ higher risk of blood clots
which oral contraceptives are monophasic?
extended cycle, continuous and sometimes monthly
what drugs interact with COCs
rifampin and ritonavir
what do you need to worry about with contraceptives that contain drospirenone like yasmin?
inc. risk for hyperkalemia
explain ulipristal?
EC
binds to progestin receptors to delay/prevent ovulation, also makes endometrium hostile
take within 5 days
need a pregnancy test before prescribing (category x)
explain levonorgestrel
EC-progestin only preventing ovulation will not terminate an existing pregnancy 3 day window OTC for > 17 yo
what do you use to treat fungal vulvovaginitis?
fluconazole (or any -conazole)
who should only use topical fluconazole?
pregnant ladies
is clotrimazole an anti fungal?
yes
