Corticosteroid risks to the eye
Cataracts and Glaucoma
Cataracts: Glucocorticoids initiate gene transcription anomalies in corneal epithelial cells that leads to over production, migration of cells and possibly to changes in protein production. Exact mechanism isn’t yet eludicated but corticos do cause epithelial cataracts and they’re usually situated on the central posterior lens.
Glaucoma: Trabecular cells swell under the influence of corticos, as do many other tissues.
Its a particular problem with trabecular cells in that their swelling closes off the canals of schlemm reducing drainage from the posterior eye to the anterior and increasing pressure in the back, squishing the retina and killing it off.
Anyone with other comorbid glaucoma risk (DM, Age, Open Angle Glaucoma) should not receive cortio rx lightly.
