Insulin - sliding scale Lithium Anticoagulants or anti platelets COCP K-sparing diuretic (spironolactone) Oral hypoglycaemic (metformin) Perindopril (or any ACEi)

Increase during surgery
A
As per ‘sick day rules’ double corticosteroid dose in patients on long term steroids

What do you do with metformin in a patient going to surgery
A
Stop - as patient is nil by mouth, can’t take metformin as it will cause lactic acidosis

When do you stop COCP and HRT prior to surgery?
A
4 weeks before surgery

When do you stop lithium prior to surgery?
A
Day before
