What are the three types of post-op haemorrhage?
Primary - intraoperatively, due to vessel damage
Reactive - within 24 hours, due to blood pressure rise and vasodilation
Secondary - 7-10 days post-op, due to vessel erosion from infection, dehiscence
Beta blockers in the perioperative period
Continue
Can be initiated
Clonidine (and other alpha-2 agonists) in the perioperative period
Continue
Do not initiate
Calcium channel blockers in the perioperative period
Continue
ACEi and ARBs in the perioperative period
Usually withhold on the morning of surgery - can prolong hypotension
Continue if used for heart failure or poorly controlled HTN
Diuretics in the perioperative period
Withhold if used for hypertension
Depends with heart failure
Nicotinic acid (B3), ezetemibe, bile acid binding resins and fibrates (non-statin lipid lowering drugs) in the perioperative period
Discontinue day before
Can cause rhabdomyolysis and myopathy
Statins in the perioperative period
