what is the function of the periodontium?
- to attach the teeth to the jaws
- to dissipate occlusal forces (tension, compression, viscous forces)
tooth mobility can be ok, unless…
- progressively increasing
- gives rise to symptoms
- causes difficulty with restorative treatment
what therapy can be used to reduce tooth mobility?
- control of plaque-induced inflammation
- correction of occlusal relations
- splinting
what is primary occlusal trauma?
injury resulting in tissue changes from excessive occlusal forces applied to teeth with normal periodontal support.
what is the response of the PDL during normal occlusal forces?
- PDL width increases until forces adequately dissipated, the PDL then stabilises
what is secondary occlusal trauma?
injury resulting in tissue changes from normal/excessive occlusal forces applied to teeth with REDUCED PERIODONTAL SUPPORT
what is fremitus?
palpable or visible tooth movement when subjected to occlusal forces?
what is bruxism?
a habit of grinding, clenching or clamping the teeth. The force generated may damage both tooth & PDLs
what is tooth migration caused by?
- loss of periodontal attachment
- unfavourable occlusal forces
- unfavourable soft tissue profile
how do you manage a patient with tooth migration?
- treat the periodontitis
- correct occlusal relations
THEN:
- accept the position of teeth and stabilise
OR
- move teeth orthodontically and stabilise
