perio Flashcards
explain gingivitis to a patient
inflammation of gum tissue which supports teeth. Main cause is plaque (mix of bacteria and food debris)
may be no symptoms or may notice bleeding, swollen, sore gums
Reversible with treatment and guidance
explain periodontitis to a patient
bone holding teeth in place is destroyed due to inflammation
caused by build up of plaque at gum margins
starts as gingivitis which is reversible but has progressed to periodontitis which sees irreversible damage to bone
can aim to control it with treatment
severe cases see mobile and even loss of teeth
3 patient groups at increased risk of perio
poorly controlled diabetes
family history
smokers
staging periodontitis
worst site on x-ray - assesses severity
I - <15% mild
II - coronal 1/3 moderate
III - mid 1/3 severe
IV - apical 1/3 very severe
grading periodontitis
assesses rate of progression
A - slow <0.5
B - moderate - 0.5-1
C - rapid - >1
%bone loss at worst site / patients age
defining extent of periodontitis
localised <30% teeth
generalised >30%
Molar incisor pattern
stable periodontitis
BOP <10%
PPD <_4mm
no BOP at 4mm sites
periodontitis in remission
BOP >10%
PPD <_4mm
no BOP at 4mm sites
unstable periodontitis
PPD >_5mm or >_4mm with BOP
acronym for giving OHI
TIPPS
Talk - ask patient about current regime, ask what patient knows about disease, discuss optimal regime
Instruct - ask patient to demo current methods and instruct on areas for improvement
Practice - allow patient to demonstrate improvements and ask questions
Plan - how to integrate new techniques into regime
Support - follow up at next visits
risk factors for peri - implant disease
- history of severe perio
- poor OH/ plaque control
- smoking
- diabetes
- no regular supportive maintenance
how to assess an implant for peri - implant disease
probe at 6 points using PCP or UNC15
assess for pocket depth, bleeding and suppuration
peri implant health
- absence of clinical signs of inflammation
- BOP <_1 site
- pocket depth <_5mm
, no increases since last visit
describe peri implant mucositis to a patient
inflammation of the gum tissue surrounding your implant
may notice swelling, redness, bleeding or may notice no symptoms
reversible at this stage
describe peri implantitis to a patient
destruction of bone around implant due to long term inflammation
loss of bone is a risk to implant lifespan
refer for treatment in possible
what radiographic amount of bone loss indicates peri implantitis
bone levels >_3mm apical of most coronal portion of osseous part of implant
