What are the aims of peri radicular surgery?
Achieve apical seal
Remove existing infection (excision of apex and curettage)
What are the indications for peri radicular surgery?
Obstruction to instrumentation (eg calcification, broken instrument, dilaceration)
Root filler error (eg under filled, open apex)
Lateral canals
Poor host tissue response
Apical cysts
Post crowned tooth with endo failure
Lateral perforation
What are the 3 types of flaps used in peri radicular surgery?
Semi lunar flap
2 sided

What instruments are used for root end preparation?
Rotary bur and ultrasonic
What is used for retrograde root filling?
Amalgam
Zinc oxide eugenol (eg kalzinol)
MTA
What can cause failure of peri radicular surgery?
Inadequate root seal (eg extra root, too little apex removed, displacement of seal)
Inadequate support (eg too much apex removed, poor perio, apical third fracture)
Longitudinal root fracture
Poor healing response
Exposure of root apex
