What is PUD?
a group of gastrointestinal disorder ,the variety based on the degree of gut.
What is the cause of PUD?
They are caused by imbalance between aggressive and defensive factors.
What are the aggressive factors of PUD?
H.Pylor, NSAIDS , gastric acid , Pepsin and smoking
What are the defensive factors of PUD?
Mucus , Bicarbonate , Blow flow and Prostaglandines
What are the most common cause of PUD?
Helicabater pylori (H,pylori) and NSAIDS.
What are the treatment of H Pylori associated ulcers?
Antibiotics (minimum of 2 different agent ) and Antisecretory agent ( PPI or H2RA fasten healing and retrieve symptoms up to 14 days)
What are the antibiotics for H.Pylori?
Clarithromycin , Amoxicillin , Bismuth , Tetracycline , Metronizade , Trinidazole.
METRONIZADE AND TINIDAZOLE
MECHANISM OF ACTION : Inhibits bacteria cellular metabolic process.
ADVERSE EFFECTS : Nausea and vomiting
OOTHER CONSIDERATION : Avoid in pregnancy and alcohol
CLARITHROMYCIN
MECHANISM OF ACTION : Inhibits bacterial protein synthesis
ADVERSE EFFECT : Nausea , Diarrhea , change in taste
OTHER CONSIDERATION : N/A
AMOXICILLIN
MECHANISM OF ACTION : inhibits bacterial cell walL synthesis.
ADVERSE EFFECT : diarrhea
OTHER CONSIDERATION : Low resistance rate , work better at higher PH
BISMUTH
MECHANISM OF ACTION : Disrupt bacterial cell wall
ADVERSE EFFECT : Black discoloration of tongue and stool.
OTHER CONSIDERATION : N/A
TETRACYCLINE
MECHANISM OF ACTION : inhibits bacterial protein synthesis
ADVERSE EFFECTS : stain developing on teeth and skin
OTHER CONSIDERATION : Do not use in pregnant and young children.
