What is a penile fracture?
Traumatic rupture of the corpus cavernosa + tunica albuginea in an erect penis
What can cause a penile fracture?
Blunt trauma - where penis is violently deviated away from its axis - when erect
- this can be from sexual intercourse
How does a penile fracture present?
Popping sensation with immediate pain and swelling Firm immobile haematoma may be palpated Discolouration - secondary to haematoma Deviation to one side “Aubergine appearance”
How is a penile fracture managed?
Analgesia
Anti-emetics
Surgery
- circumferential incision - penile skin degloved - removal of haematoma. The repair the tear
Abstinence from sexual activity for 6-8 wks post surgery
What are some complications of penile fractures?
Penile parasthesia
Dyspareunia - painful erection
Erectile dysfunction
What else is important to ask if someone presents with a penile fracture?
Ask about haematuria - urethral injury
