Peds (Pediatric Hip Conditions) Flashcards
Legg-Calvé-Perthes Disease
Osteochondrosis: a disease of the growth or ossification centers in children which begins as a degeneration or necrosis followed by regeneration or recalcification
Self-limited disease of hip produced by
ischemia, varying degrees of avascular
necrosis of femoral head and
spontaneous regeneration
Legg-Calvé-Perthes Disease Incidence
-Most commonly boys 5-7
-Active, skeletally immature boys
Legg-Calvé-Perthes Disease Intervention
Goal: containment of femoral head so it
regenerates smoothly with good shape
Physical Therapy -ROM: especially to gain hip abduction, internal rotation -Gait training with device or post-op -Promote symmetry -Pain management
Slipped Capital Femoral Epiphysis (SCFE)
Displacement of femoral head on femoral
neck; head inferior and posterior in relationship
to femoral neck
SCFE Incidence
-Boys:girls, 2:1, even higher in African American boys
-Tend to be obese, skeletally immature males,
around time of puberty (11-14)
Legg-Calvé-Perthes Disease Impairments
Impairments
-Pain often in knee or groin
-Limited hip ROM: abduction, internal rotation
- Limp
Functional limitation -Gait -May be inconsistent Long-term outcome: approximately 50% will have degenerative hip disease by 5th decade
SCFE Impairments
- With flexion, hip moves into ER -↓ hip flex, abd, int rot -Limp - Pain -Shortening of affected limb
SCFE Intervention
-Range of motion
-Gait training: non-weight bearing
preoperatively
-Promote symmetry
