Infant under 3 months, fever over 100F

Do breastfed infants have BMs every day?
They may not - breast milk doesn’t make for a great deal of waste.
Describe Intestinal Rotation
It is usually a congenital disorder of the intestine that predisposes the intestines to kinking (i.e. volulus)
It usually presents in the 1st year of life with colic, bilious (usually) vomiting and FFT if acute but milder forms may be misdiagnosed into adulthood until a full on volvulus occurs
Upper GI Endoscopy is the imaging test of choice
Intestinal decompression, stabilization and then surgery
Sudden onset bilious vomiting, think
Obstruction, usually small bowel
Malrotation imaging test
UGI endoscopy
Air Enema is the 1st line Rx
Intussusception
Intermittent “knees drawn up” belly pain in infant with current jelly stool:

Telescoping of one intestinal portion inside another - big trouble
Most common shock in Peds
Dehydration/Hypovolemia
BEWARE!!! PEDS Bps stay high until 40% of blood is lost then they CRASH!!
Assist Respirations + Get IV fluids into any shock kid ASAP
