Pediatrics: Interventions: Duchenne's Muscular Dystrophy Flashcards
1
Q
Duchenne’s Muscular Dystrophy
A
- X linked
- Recessive
- Inherited by boys
2
Q
Duchenne’s Muscular Dystrophy: Dystrophin
A
- Missing
- Causes increased permeability of sarcolemma and destruction of muscle cells
3
Q
Duchenne’s Muscular Dystrophy: Collagen and Adipose
A
- Laid down in muscles
- Causes psuedohypertrophic muscles
4
Q
Impairments: Diagnosis
A
- Diagnosis made in 3-6 years of age
-
5
Q
Impairments: Early Signs
A
- Progressive weakness
- Delay in walking
- Difficulty rising from supine or sitting
- Frequent falls
- Proximal weakness before distal weakness
6
Q
Impairments: Gower’s sign
A
- Child must use arms to “walk up” legs to rise to standing
7
Q
Impairments: Contractures and Deformities
A
- Heel cords
- TFL
- Lumbar Lordosis
- Kyphoscoliosis
8
Q
Functional Limitation
A
- Milestones delayed
- Ambulation ability will eventually be lost, necessitating eventual use of a wheel chair
- Cardiopulmonary limitations
9
Q
Interventions Goals and Prognosis: Red Flag
A
- Do no over exercise children with DMD at a maximal level.
- This can lead to over work injury due to musculoskeletal impairments
10
Q
Medical and Surgical Management
A
- Steroids for decreasing pulmonary dysfunction
- Antibiotics for pulmonary infections
- Orthopedic surgery for scoliosis
- Muscle lengthening of the gastrocnemius
11
Q
Becker’s Muscular Dystrophy
A
- Slower variant to DMD
