What are some tips to approaching patients less than 6y/o?
Always knock
Open door slowly so you don’t knock a kid over
Leave younger ones with parents at first, move slowly
Be midnful of “Stranger Anxiety” between ages 6m-3y.
What are some features of pediatric history that should be noted?
age specific-diet, elimination, sleep patterns
pregnancy, delivery and newborn histories
developmental history
How should pulse and RR be obtained in patients under 6 y/o?
listen and count for a full 30s and then double
can be variable. If very high, listen and count the full minute
Why is observation important in pediatrics?
can give a lot of information about general health status and development without scaring the kid or having to touch them
What are some important SDOH questions to ask patient/parents?
living/working/playing environment can have a role in health, substance abuse, education challenges and underemployment
how do children with COVID-19 present?
many will have no sx
if present, mild with low grade fever, fatigue, cough, loss of taste/smell
can lead to MIS-C which is life threatening (looks like Kawaski)
What is the pupose of well-child visitis?
prevention
tracking growth and development
parents can ask questions
physicians can address abnormal findings
establish a medical home
What is the resource Bright Futures?
a list of recommendations for prevention for infants, children, adolescents, and their families
When seeing a newborn, make sure to address what?
Ask mom about post-partum depression
What is important to keep in mind when getting a history from pediatric patients
A lot of the history comes from parents who know their child best and will have best informtion
Be mindful of the hyper and hypovigilant parents
What is important history to obtain in pediatric patients, esp if younger than 5 and/or presenting with developmental or syndromic features?
pregnancy history including medications, complications and prenatal care
What is the APGAR score?
assigned to newborn at 1- and 5-minutes after birth
rated based on heart rate, breathing, tone, reflex, and color
What is something to consider obtaining in history, in addition to pregnancy history?
post-natal history
(was baby in NICU, if went home with mom, if passed oxygen saturation test, etc.)
What should be discussed during a developmental hisotry screening?
discuss age-appropriate milestones
What longitudinal assessments should be made during physcial exam?
height, weight, bp, and sexual maturity
