RDS - Respiratory distress syndrome s/s?
nasal flaring, grunting, restraction of chest wall, RR 60-120, Tachypnea, Cyanosis
RDS signs are with the first?
24hrs
RDS txn?
1st supplemental O2 - can be mechanical vent - CPAP
2nd minize handling, disturbance
3rd Temp is important
4th Blood glucose monitor
5th arterial line to monitor BP, ABG
BPD - Bronchopulmonary Dysplasia occurs in ?
low wgt babie 1000-1500g
Severe BPD sign?
Clubbing of nails
BPD S/S?
tachycardia, rapid shallow breaths, chest retraction, cough, poor wgt gain/failure to thrive
BPD can cause?
pulmonary hypertension which can cause Right sided HF - which can cause periorbital edema and hepatomegaly
BPD Txn?
- Nutrition- 120-160 cal/kg/day
- Supplemental Electrolytes b/c they may take loop diuretic for Right sided HF
- Mechanical Ventor (PPV) - Oral hygiene
- Meds- Inhaled Corticosteriods, broncodilators, and Caffeine citrate - treats apnea of prematurity and Vit A
Causes of Acute Epiglottitis?
Not getting HiB vaccine
Epiglottitis is usually in what age?
2-7 years
Epiglottitis S/S?
HIGH FEVER, Sore Throat, Irritability, Stridor, Severe respiratory distress, Anxious, DROOLING , Sitting in tripod, DYSPHAGIA
Epiglottitis may present with?
pneumonia, Otitis, Meningitis, Sepsis
Epiglottitis is?
LIFE Threatening
Epiglottitis nursing management?
- Do Not attempt to visualize the throat
- Do not leave child unattended
- Do Not lay supine
- Provide 100% OXYGEN - Trache may be necessary
- Ensure Emergancy equipment
Epiglottitis Txn
- Oxygen
- Racemic Epinephrine
- Corticosteriods
- Rifampin
