Pediatric IV Fluids Flashcards
What is daily caloric expenditure calculation for child <10 kg, >10kg - 20kg, and >20kg - 80kg?
- <10 kg: 100 kcal/kg
- >10 to 20 kg: 1000 kcal for first 10 kg + 50 kcal/kg for any additional
- >20 kg to 80 kg: 1500 kcal for first 20 kg + 20 kcal/kg for any additional
What is calculation for insensible losses?
- Insensible = 45 mL per 100 kcal of energy expended
- Sensible = 55 mL per 100 kcal of energy expenditure
Using Holliday-Segar 4-2-1 method what is hourly fluid requirments for pt who is <10kg, 10-20kg, and >20kg?
- <10kg = 4 mL/kg/hr
- 10-20kg = 40 mL + 2 mL/kg for every kg >10kg
- >20kg = 60 mL + 1 mL/kg for every kg >20kg
Using Holliday-Segar 100-50-20 method what is daily fluid requirments for pt who is <10kg, 10-20kg, and >20kg?
- <10kg = 100 mL/kg/day
- 10-20kg = 1000 mL + 50 mL/kg/day for every kg >10
- >20kg = 1500 mL + 20 mL/kg/day for every kg >20
What are the mEq of Na, Cl, and K per 100cc of water that should be given?
- Na and Cl –> 3 mEq per 100cc water
- K –> 1-2 mEq per 100cc water
For rehydration boluses in a child you should always use what?
Isotonic fluid: LR’s or NS
How many mEq/L of Na and Cl are found in 0.9% NS?
154 mEq/L of Na and 154 mEq/L of Cl
Which 4 clinical signs are most diagnostic of dehydration in a child?
- Capillary refill >2 sec.
- Dry mucous membranes
- Absent tears
- Abnormal general apparance
A 20kg child is determined to be 10% dehydrated, how much of a fluid deficit are they in?
0.10 x 20kg = 2kg —> 2kg x 1000 = 2000cc of fluid deficit
In rehydration of a child, how many cc/kg of fluid bolus are given?
- 10-20 cc/kg
- For 20kg child this is 200cc - 400cc
When rehydrating over 24-hrs how is the amount of fluid distributed?
- Half is given during first 8 hours
- Last half given over the next 16 hours
