Febrile child

A

Vital signs
Growth parameter - Ht, Wt, Head circumference
GI - environment, child - comfort, alert, interaction, nutrition, normal or Dysmorphic
GI - Head to toe, ID Dysmorphic features
Eyes - Conjunctive pallor, jaundice
Mouth - central cyanosis, hydration, infection, tonsils
Neck - LN and thyroid
Chest - Central CRT, Resp exam, and CVS
Hand - Clubbing
Back - scars especially for Neuro
Toes - same as hands.

Resp Exam

A

Inspect
- WOB - RR, Accessory muscles, head bobbing in under 1yr, recession of all types
- Sound
- Shape
- Scars
- Billaterally equal
Palpation
- Expansion in the 3 zones both anterior and posterior
- Bony tenderness
- Trachea divation
- Apex beat - in the 4th intercostal space until 8-10 yrs
Percussion - bilarterally and anterior and posterior
A - both. Be able to draw the sounds.

A

800 - 2000ml per day
1-2ml/kg/HR
0.5ml/kg/HR is oligouria
