-HEENT: NC/AT; EOMI; PERRLA; no cyanosis, no icterus, no conjunctival pallor; moist mucous membranes; no tonsillar erythema, enlargement or exudates.
Neck Short PN
-NECK: supple, no thyromegaly, no LAD.
Chest/Respiratory Short PN
-CHEST/RESP: CTAB; no rales, rhonchi, wheezing, or rubs; no tenderness on palpation.
CVS Short PN
-CVS: RRR, S1/S2 WNL, no M/R/G, no JVD, PMI not displaced
Abdomen Short PN
-ABD: soft, NT, ND, BS+ in all 4Q; no HSM; no palpable masses; neg. for: rebound tenderness, psoas etc..
Musculoskeletal Short PN
-MSK: motor strength 5/5 in all 4 ext. full ROM at joints; special tests..
Extremities Short PN
-Extremities: pulses +2 b/l, no cyanosis, clubbing or edema.
Neuro Short PN
-NEURO: Alert and oriented x3; cranial nerves 2-12 intact; DTRs (biceps and patellar) WNL; Motor and sensation intact in all 4 ext.; special tests.
