Why do women with PCOS have periods of amennhorrea then have heavy periods.

A

There is prolonged exposure to estrogen which causes continued proliferation of the endometrial tissue. The endometrial tissue is usually shed due to a progesterone withdrawal but this does not occur and as such the monthly shedding does not occur. Periods of anovulation cause amenorrhea

Eventually shedding will occur due to necrosis of the endometrium as it proliferates and outstrips its blood supply. This causes a lot of bleeding.
