nonfluent aphasia
comprehension good
assciated with contralateral hemiparesis if motor strip is involved
wernicke’s aphasia
superior temporal gyrus
speech fluent but nonsensical
poor comprehension and awareness
symptoms if CNS affected
increased spasticity/tone and reflexes
pyramidal pattern - arms have extensors weaker than flexors
legs have flexors weaker than extensors
opposite side of brain involved
same side of spine involved
brain- hemisensory and sensory level of spinal cord
peripheral nervous system symptoms
decreased tone and reflexes
peripheral neuropathy- distal weakness
root/plexus/mononeuropathy- dependant on innervated muscles
plantar response- none/flexor= normal
NMJ symptoms
ptosis/opthalmoplegia
decreased/normal tone and reflexes
fatiguablity
normal plantar response
muscle symptoms
proximal wasting symmetrical
decreased/normal tone and reflexes
normal plantar response
NMJ diffrential diagnosis
lambart- eaton- pre synaptic - autonomic features
Myaestenia gravis- post synaptic - occular
peripheral neuropathy diffrentials
acute- GBS Guillain-Barré syndrome
chronic- diabetes/alcohol
