Pain
A
- Character (mild,moderate,severe/intermittent, continuous,itching,throbbing pain,sensitivity)
- Onset (since when)
- Duration (lasts for how long, sec,min,hours, days etc)
- Aggressive factors (increased when eating,brushing, cold or hot drinks etc)
- Relieving factors (decreased with … e.g brushing or wooden sticks to stimulate bleeding)

Lymph nodes :
- Submental
- Submandibular
- Pre-auricular
- Posterior auricular
- Occipital
- Submental
- Parotid
- Tonsilar
- Superficial cervical
- Deep cervical
- Supraclavicular

Possible Findings when examining lymph nodes
A
- Palpable or Non-palpable
- Tender or non-tender
- Firm or soft
- Mobile or fixed etc
NB Always palpate

Extra oral examination focus on:
A
- Eyes: Especially the colour of the sclera
- Skin colour: If pale or yellowish
- Face symmetry
- Lymph nodes
- Hands (fingers, finger nails etc)
- Feet (Toes, toe nails etc)
- Lips
- Chin

- Anaemia
- Clubbing
- Cyanosis
- Lymphadenopathy

INTRA-ORAL EXAMINATION
A
- Hygiene (Good, fair or bad) (depending on amount if plaque or calculus present in patients mouth)
- Palate (Hard and Soft palate)
- Buccal mucosa
- Labial mucosa
- Buccal sulcus
- Labial sulcus
- Floor of the mouth
- Tongue (Dorsum and Ventral surface)

Possible diagnosis
A
- Anything that was noted in the oral cavity
E.g Gingivitis, periodontitis, caries, ulcer etc - Tumors
- Edentulous
- Partially edentulous
- Xerostomia
