16
Q

Diagnosis?

A

Identify pt’s learning needs based on the three domains of learning

A

Establish learning objectives (behavioral terms) and identify teaching method

18
Q

Implementation?

A

Q

What is the teach back method?

A

Evidence based approach used to assess whether or not a patient understands info

ex. I’ve given you a lot of info to remember. Please explain to me so that I can be sure that I gave you the info that you need to take care of yourself.

Use handouts, pictures, and models to reinforce teaching

Chunk and check

IS NOT INTENDED TO TEST PATIENT BUT TO CONFIRM HOW WELL TAUGHT INFORMATION IS

A

Determine who is the patient’s primary caregiver

Assess and determine perception and understanding

Create a teaching plan

A

Not aware, uninformed, no intention to change

25
Q

What is contemplation? (TM)

A

Aware problem exists are thinking about changing

26
Q

What is preparation? (TM)

A

Intention to take action to change

Q

What is action? (TM)

A

Makes modifications in their behavior

Q

What is maintenance? (TM)

A

Have made modifications, prevent relapse

Q

What is termination? (TM)

A

100% self efficacy, no temptation to relapse

30
Q
