HPCAA -

What is it and what impact has it on MRTs

A

The purpose of this act is to protect the health and safety of the public by ensuring all the practitioners are fully competent in their profession. For MITs this is governed by the MRTB, they ensure MITs are all competent by ensuring they –
• Have an annual practicing cert (APC)
• Meet the continuing professional development hours
• Following code of rights CoR
• Following code of ethics CoE

MRTB vs NZIMRT - why are both important

A

Medical radiation technologist board - governing body/registration board, duty to ensure MITS are fulfilling duties to meet HPCAA. Handles registration, complaints and disciplinary action.

New Zealand Institute of Medical Radiation Technologists - professional body for MRTs. Provide a “voice” for the profession, provide educational opportunities, have a CPD programme to support requirements to practice.

What is it?

A

The act of the health and disability commissioner is to promote and protect the rights of health and disability services consumers and facilitating the fair, simple, speedy and efficient resolution of complaints.

A

Make an informed choice and give informed consent

To support - the right to have a support person accompany one to examinations/appointments

To complain - one should not be disadvantaged for making a complaint, they should be kept informed of the progress and outcome of complaint.

Appropriate standard - receive necessary care and skill required to meet needs

Fully informed - services only be provided once you have made an informed choice and given informed consent.

Code of ethics -

What is it and list the ethical principles

A

Code generated by the MRTB/NZIMRT. Aim of the code is to assist MRTs in maintaining the highest standard of ethical conduct.
Standards include -
* MRTs will act in a manner to justify public trust and confidence
* MRTs are committed to provision of the best possible service to patients. By maintaining CPD
* MRTs committing to use an ALARA exposure
* MRTs protect pt rights

Autonomy - having respect for the individual and their ability to make decisions regarding their health and future
Non-maleficence - actions intended not to cause harm to the patient or others
Beneficence - actions intended to benefit the patient
Justice - being fair and just to the wider community in terms of consequences of an action
Fidelity - notions of loyalty, faithfulness
Veracity - truth telling, honesty

Mumps protocol

A

If patient has mumps ideally a male MIT should not be involved as contracting this virus may cause sterility.

Mumps is droplet isolation which requires MIT to use surgical mask, face shield and gloves. Wash all surfaces that patient has had contact with.

Isolation period is 9 days after initial inflammation

MRSA - what it is, what is the protocol, room prep/MRT prep

A

Methicillin resistant staphylococcus aureus - bacteria which is resistant to a lot of broad spectrum antibiotics.
MRSA is a contact precaution.
Decided whether patient should be xrayed in room or ward. When patient arrives should be covered in a blanket.

Room prep - 
Use room 3
Remove unnecessary equipment 
Place yellow infectious bag in room  
Clean/dirty person
Detector covered in plastic sleeve 
Doff into yellow bag 
Once patient leaves - change gloves and clean all horizontal surfaces 
Wash hands afterwards

TB protocol

A

Tuberculosis is an airborne infectious disease. So staff need to wear an N95 mask and a face shield, they may also want to wear a gown and gloves.

The room should be left for 30mins to allow particles to drop and then all surfaces should be wiped thoroughly.

A

ABC on patient
Push green button and yell for help, tell someone to call 7777 and send for adult crash team
Start cardiac resus and send first person for crash trolley
Ensure crash team directed to area

If paediatric call 7777 and send for paed crash team

Emergency buttons in each of the xray rooms behind control panel.
If CT button in workroom on wall

Remove myself and others from the area

Contain fire by closing doors and getting to fire cell

What is the difference between infective and protective isolation?

A

Protective isolation is for patients who have decreased immunity for example neutropenic patients. Patient needs protected from illness. Ideally they would be in a positive ventilation room.

Infectious isolation is for patients who are contagious e.g. MRSA. The patient is infectious and we need to protect ourselves from them. Ideally they would be in a negative pressure ventilation room.

ABCs - airway, breathing, circulation

If pt fainted loosen clothing and raise feet - call for help

A

Reassure patient that i am going to find out what is happening.
Find fire warden in department and await instructions.
If able to continue, finish xrays get patient ready to evacuate.
Close doors, shut equipment off.
If able wheel patient into the fire cell with everyone else, if theres no time close all doors to the xray room and explain to patient what happens next. Get firefighters to sort patient out first.

Plan and prepare

Spinal lift -
Three people either side of patient
One person at head and one at the foot of patient
The three people on the body will link hands with person opposite
The person at the head will be in charge of calling out instructions/timings etc

Scoop work -
Scoop is a stainless steel frame that clips together at the top and bottom.
Pt supported under shoulders and pelvis
Pt log rolled to one side then the other and half of scoop placed under each time
Cannot xray through

You are xraying a patient who is in spinal precautions when they say that the are going to vomit. What do you do?

A

There should be a nurse there to guide a log roll
Suction ready
Call for assistance
Towel/bowl
Remember to keep the neck stable at all times

You are xraying an 80 year old who is in for c-spine. They are very unsteady on their feet. How would you carry out this examination?

A

Sit patient on a stool
Change to broad focus
Dont expect patient to hold weights for too long or at all

What are the departments protocol when xraying covid patient?

A

Covid is an airborne and contact precaution. Which means MITs need to wear gown, N95, face shield, gloves. x2 MITs clean/dirty

If a patient is a high risk covid patient we would usually do a portable to R1 where they house the covid infectious patients. Take the portable machine into the room with the bare minimum on it - 1 slide shet, 1 plastic detector cover etc
When come back out of the room, we need to doff correctly. And then clean all equipment with suggested wipes.

If we to xrays in department we use room 3 as has best airflow - negative pressure. Cover equipment. Wait 30 mins once finished and then clean all horizontal surfaces.
