Man presents with lethargy and jaundice and macrocytic anaemia. He has a raised PT. LFTs show raised ALP and GGT, (alcohol excess). He is vomiting fresh blood.
Oesophageal varices
“Hunger” pains ie worse before meals. Dyspepsia and burping. Upper GI pain.
Duodenal ulcer
67 year old woman with severe LIF pain. Vomiting, pyrexial. Guarding but no peritonism.
Acute diverticulitis
Asymptomatic lesion found in the ileum about 60cms from the ileocaecal valve. Diagnosis of exclusion. Demonstrated via a radionuclide scan using technetium-99.
Meckels diverticulum
Recent pulmonary oedema following a surgery (anaesthesia) with no past medical history.
Iatrogenic ( Myocardial ischaemia results in underperfusion of the kidneys resulting in oliguria and fluid retention)
Young Man with increasingly (4/7) SoB, severe chest pain and fever. Previously fit and well. He has a gallop rhythm but no murmurs. Normal heart size. ECG shows extensive ST elevation in anterior and inferior leads without Q waves.
Myocarditis
38 year old woman with 2/7 Hx of abdo pain and vomiting. Epigastric tenderness and very high serum amylase. Persistently hypoxic and deteriorating rapidly with pulmonary oedema. PaO2 of 5.1 kPa despite 60% O2. Low albumin.
ARDS (due to capillary leakage)
