What is resistant hypertension?
failure to reach goal BP in pt’s taking full doses of an appropriate 3 drug therapy regimen that includes a diuretic
What are some reasons for resistant hypertension?
- improper BP measurement
- drug-induced
- assocaited conditions
- identifiable causes of secondary hypertension
What would you aska pt who comes in and their BP in not in range?
Have you been taking the medication
What is important to remember about the older population and hypertension?
- increased incidence with age
- systolic HTN is common >50 years
- white coat HTN is common
- altered drug metabolism/excretion
- wide gap in Korotkoff sounds
- increased risk for ortho hypotention
- postprandial hypotension
What is the BP goal for people > 60?
150/90
Explain how to measure for ortho hypotension?
-BP and HR supine, sittin, and standing
-Measure within 1 to 2 minutes of position change
-positive:
decrease of 20 in systolic
10 or more in diastolic
increase of 20 beats/min or more in HR
