Family and Medical Leave Act of 1993:
Title 29, Part 825 of the Code of Federal Regulations that entitles qualified employees up to __ weeks of unpaid leave per year for the birth or adoption of a child, placement of a child with an employee for foster care, to care for a spouse or an immediate family member with a serious health condition, or when unable to work because of a serious health condition.

FMLA
Leave of Absence:
Leave of Absence is an approved absence without pay and may be granted for a maximum period of __ one year, including any period of Mandatory Leave, and a minimum period of __ pay period.

FMLA Eligibility:
- Must be employed by the County for at least 12 months.
- Must be employed by the County for 1,250 hours or more during the 12 months prior to the commencement date of the requested leave.
- Must not have taken ____ of mandatory leave as provided for under FMLA within the same calendar year as the commencement date of the requested leave.

Family/Medical Leave:
To request Family/Medical Leave, employees shall submit requests to the __ for review before submitting an action memorandum to the Director for approval, via appropriate channels.
Personnel Management Bureau
Employees seeking Family/Medical Leave must provide the Director at least __ days advance notice before the leave is to begin if the need for leave is foreseeable. If __ days notice is not practicable, notice must be given as soon as possible.

Employees are entitled to 12 weeks of Family/Medical Leave in accordance with Miami-Dade County Leave Manual, Section 27, Mandatory Leave.
This does not preclude an employee from requesting, and being approved for, a ___ in excess of 12 weeks.

Leave of Absence
Service Member Family Leave:
The National Defense Authorization Act for 2008 permits a spouse, son, daughter, or next of kin to take up to __ work weeks of leave.
A serious injury or illness for purposes of the act is defined as an injury or illness incurred by the member in the line of duty while on active duty.
**The 26-week leave entitlement is not in addition to the 12 weeks currently available to qualifying employees under previous FMLA provisions. **

Leave Without Pay:
If an employee requests leave without pay, a MDPD ________ (Annex B) must be completed.
Leave Without Pay is used for an authorized absence _when no appropriate accrued leave, Birthday Holiday, or Floating Holiday are available. _
